There are many different types of claims that an individual can file with the Vermont Department of Labor.

Unemployment Insurance Program

If you are interested in filing an Unemployment Insurance claim, hours of operation, contact and filing information is provided.

File a weekly claim

Workers' Compensation Program

If you have been injured at work and your employer has not filed a First Report of Injury you should file a Form 5.  You can also contact Monique Payette at (802) 828-2149 or monique.payette@state.vt.us for further information.

Wage and Hour Program

If you are interested in filing a claim for wages, you may fill out the online wage claim form  ,or use the mail in format of our wage claim form. You can also contact us at (802) 828-0267
