Forms for Single Employer Programs
Form #: 263 - Pre-Registration
When to use this form: Prior to registering an apprentice in RAPIDS, use this form to collect all necessary information, and to ensure the apprentice has received the proper notice that they are to be provided a copy of their standards. This document should be printed and signed by the apprentice, then uploaded to the apprentice's record in RAPIDS.

Form #: 274 - Change in Status (12/22 v 2)
When to use this form: This form is to be used when the sponsor is changing the status of any apprentice in RAPIDS. A "wet" signature is required on this form. The scanned version of this file should be uploaded to the apprentice's record in RAPIDS before the action is processed.

When to use this form: Prior to registering an apprentice in RAPIDS, use this form to collect all necessary information, and to ensure the apprentice has received the proper notice that they are to be provided a copy of their standards. This document should be printed and signed by the apprentice, then uploaded to the apprentice's record in RAPIDS. This form is intended to make it easier for the program sponsor to complete data entry for the apprentice.

Affidavits - Verification of Prior Experience
These forms are used to assist an apprentice in transferring from one sponsor's program to another sponsor's program in the same occupation by formally documenting the number of on-the-job training (OJT) hours accrued by an apprentice under different employers and supervisors. The number of hours that can be transferred to a new employer is defined by State and Federal law, and can be further defined by each employer.
Occupation: Electrician
When to use this form: When an apprentice has prior OJT hours, is in the process of changing employers, and wants formal verification of their OJT hours under the licensed Master electrician.
