Request for Separation Forms
*Please note, receiving sick pay does now disqualify individuals from receiving unemployment insurance benefits
Separation forms are mailed to the most recent employer for an individual who has established a claim for unemployment insurance benefits with the Vermont Department of Labor. Forms are mailed in order to determine the reason for the claimant’s separation.

Additional and Extended Benefits
There are currently additional benefit programs for claimants in Regular Unemployment Insurance once their traditional 26 weeks of benefits have been exhausted.
