Request for Separation Forms

*Please note, receiving sick pay does now disqualify individuals from receiving unemployment insurance benefits

Separation forms are mailed to the most recent employer for an individual who has established a claim for unemployment insurance benefits with the Vermont Department of Labor. Forms are mailed in order to determine the reason for the claimant’s separation.

Extended Benefits

There are currently additional benefit programs for claimants in Regular Unemployment Insurance once their traditional 26 weeks of benefits have been exhausted:
  • Pandemic Emergency Unemployment Compensation (PEUC) provides an additional 13 weeks of benefits to claimants as a program under the federal CARES Act. PEUC went into effect April 5, 2020 and will be available to claimants for 13 weeks or until the benefit week ending December 26, 2020, whichever is sooner.
